Call or submit a non-urgent administrative request. Our staff will review scheduling availability and next steps.

Provide Registration Information

Our staff will let you know which demographic, insurance, medication, and prior-record information is needed before the visit.

Attend Your First Visit

Your visit will take place through the Wellness Center at Splendido at Rancho Vistoso. Your physician will review your history, current concerns, medications, preventive needs, and goals for ongoing care.

Prepare for Your Visit

Helpful items to have available.

  • Photo identification
  • Insurance cards, if applicable
  • Current medication list, including doses
  • Allergy information
  • Names of important specialists
  • Relevant recent hospital or specialist records, if available
